POLICIES AND CONDITIONS

1. Membership Eligibility Ages

· Children ages 14-17 may workout without a parent or guardian during staffed hours only

· Any member under 18 yrs of age must be accompanied by a parent or guardian after hours

· No person under the age of 8 is allowed in the facility at any time unless approved by management

2. Tanning

· Tanning may be used during staffed hours only

· Tanning packages may not be shared

· Goggles mandatory

· Maximum exposure time 10 min

· No use of lotions bought elsewhere unless approved by management

· Minimum age to tan is 14 yrs

3. Personal Training

· One hour free personal training every eight weeks with monthly EFT accounts only

· 1 month members will receive a free “orientation” from employees at the availability of the employee during staffed hours

· There will be no refund or credit given for any or all personal training sessions not used during your membership

4. EFT Payment Due Date:

· All EFT payments from credit/debit cards, checking or savings accounts are due on the 1st working day of each month.  All returned credit/debit card, checking or savings account funds will be electronically resubmitted within 3-10 business days along with a $25 service charge.  All access keys/codes will be inactive until payment is made on your account.

· Holds may be put on your EFT payments for up to 60 days, fees may apply.

5. General Rules

· Every member must pay 24-hour access initiation fee and each have a key/code of their own

· You must use your own key/code to enter the facility, if your key is lost you must purchase a replacement code

· You may bring guests during staffed hours only: a guest fee will be charged at the time of service

· Lockers are provided for daily use, any locks left on lockers for more than 2 days will be removed by management

· Street shoes are not allowed on workout floor at any time, you must change your shoes before going onto rubber flooring.

· Proper attire must be worn when working out:  i.e. tennis shoes, shorts, sweat suits and t-shirts (no jeans, boots, cutoffs or swimsuits allowed)

· Non-use of the facility does not entitle you to any refund or credit for your membership term

· Your monthly EFT membership payments will continue to accrue if for any reason we are unable to collect funds from your credit/debit card, checking or savings account.  Your membership will not be cancelled due to non-payment until we receive a 30 day written notice by certified or registered mail or you come in to the facility to sign the proper cancellation forms.  Any past due accounts will be subject to collection procedures.

· Absolutely no use of the free-weight bench press after hours.

6. Household addition rules:

· Spous

es/significant others and children from the same household address ages 8-19 may be added onto your EFT account. 

· Roommates and friends may NOT be added onto your account even if they live in the same household, they must have their own membership.

· Add-ons to your EFT account may be made at any time: to remove members or tanning from your account we must receive a 30 day written notice prior to your next payment date in order to make any changes to you EFT account

7. Member Discounts

· Corporate accounts are available and can be set up by contacting the main office

· Student/Senior(60+) discounts are only valid if proof of school or age is provided

8.  24-hour Access Rules

· Do not allow anyone into the facility with you at any time for any reason.  This includes spouses, children and family.  Children under 8 are not allowed in the facility at any time. 

· I understand that if I have a balance on my account my key will be inactive until payment is received.  Ultimate Fitness will not be liable for any injuries, loss or theft of personal items during staffed and un-staffed hours.

· Memberships are non-transferable and non-refundable: any members sharing access keys/codes or bringing in other non-members with them during un-staffed hours will be subject to a $75 fine for these activities. 

· Ultimate Fitness Inc. has the right to refuse service to or discontinue/cancel memberships at their discretion for any person or member with no refund of any payments made.
