To cancel your EFT transactions you must give a full 30 day written notice before your next scheduled payment date by filling out a contract termination form at any Ultimate Fitness location or by certified or registered mail sent to: Ultimate Fitness, 1910 Geneva Street, Sioux City, IA 51103. Verbal notification, faxes and/or emails will not be accepted.
